How to Map Information from Online Forms to Alerts in Your Practice Management System


Step 1: Access the Forms Page


1. At the top of the PbN interface, find and click on the Forms page.

2. On the left side, select Lists.


Step 2: Viewing and Mapping Options


1. Here, you can view and map information for Medications, Diseases, Allergies, and Referrals.

2. When a patient completes an online form, the mapped alerts are automatically written back to your practice management system (PMS).


Step 3: Using Auto Map


1. For first-time setup, click the Auto Map button to save time.

2. Auto Map searches for matches for each alert in PbN from your PMS and writes back the selected alerts.

3. Review the mappings to ensure nothing is missed or needs adjustments.



Step 4: Adding Custom Categories


1. At the top of the Lists page, click +Category to add new sections for medications, diseases, or allergies.

2. Note that default categories cannot be altered, but custom sections can be created.



Step 5: Adding Alerts


1. Click the + sign to the top right of each category to add more alerts to PbN.

2. Patients can select these alerts when completing online forms.

3. Map the alert to your PMS by clicking anywhere in the alert field.



Step 6: Managing Alerts

1. Under Actions, you can:
  • Enable: Make an alert visible in PbN.
  • Inactivate: Hide an alert on the online forms.
  • Delete/Edit: Rename or remove an alert.


Step 7: Referral Options


1. Under the Lists page, find Referrals.

2. If active, patients can indicate how they heard about your practice on the online forms.

3. Use the first three checkboxes to allow patients to write in "Another Doctor," "Another Patient," or "Other," requiring a name.

4. Select referral sources from your PMS and use the arrows to move them to the field shown to your patients.

5. Edit the names of referral sources as needed.



Step 8: Setting Custom Questions

1. You can set custom text and date-type questions for the Medications, Diseases, and Allergies tab.

2. Patients will answer these custom questions when selecting an item from the list on the medical history form.
